When we ask why they didn’t seek medical help, we often hear responses like, “Doctor, we thought there was no solution to this.” “After all, we thought it was normal after a certain age; my neighbor had urinary incontinence, my sister-in-law had it, my sister had it, my friend had it,” and so on. However, this is a huge misconception. Yes, the percentage of urinary incontinence increases after a certain age.

Especially after the age of 50, almost every other patient has complaints of urinary incontinence. The fact that this occurs so frequently does not mean it is unsolvable. “Urinary incontinence is not your fate.” It is a disease with a very easy solution, and there is no need for your quality of life to be compromised unnecessarily because of it. Some women avoid going out for fear of leaking urine, avoid drinking water, and distance themselves from social life. They think, “Anyway, if I leak, I can do it at home and quickly take a shower.” Although urinary incontinence seems like a simple complaint, it is a problem that isolates, tires, and can even lead to depression in women. In fact, the solution to this problem is very simple, but it is not only seen in the elderly; it can also occur in younger individuals, although its frequency increases with age.

There are basically two types of urinary incontinence. They are so different from each other, yes, they both result in urinary leakage, they have similar complaints, but both the mechanisms of occurrence and the treatments are completely different.

In the first type of urinary incontinence, the main problem is that the reflexes that will stimulate the patient to urinate suddenly stimulate the bladder, and the patient suddenly feels the urge to urinate and rushes to the toilet, but unfortunately, leaks urine before reaching the toilet. The basic mechanism here is the stimulation of the bladder due to neurological miscommunication, so we mostly use medications, some exercises, and laser therapy in its treatment. But we can achieve a happy ending mostly through medications. The other type is completely different. The problem in the other type is not the stimulation of the bladder. The bladder is not stimulated, but there is a urethra that allows us to urinate after the bladder. The bladder and the urethra. Urine flows from the bladder to the urethra and then to the toilet. Now, in some cases, for example, age, weight, pregnancy, normal childbirth, the connection between this bladder and urethra loses its strength and becomes very loose. This time, when the pressure inside the abdomen increases, what happens? Coughing, sneezing, laughing, jumping, exercising. In these situations, when the pressure inside the abdomen increases, pressure-dependent compression occurs in the bladder, and since the connection we call the urethra is not as stable as before, urine leaks suddenly. Now the solution here is not so much medication as in the second case. Because there is no neurological miscommunication here either. The important thing here is to strengthen and restore the connection between the bladder and the urethra. Therefore, exercises that strengthen these muscles, laser therapy, and as a last resort, if we cannot solve it, surgery to strengthen and stabilize the connection of that urethra can solve this problem. In some cases, both can occur together, and this is called Mixed urinary incontinence. After evaluating the patient in detail in this case, we can apply surgery, exercises, medication therapy, or a combination of these, depending on which finding is more severe. It is a disease with a solution in every case; do not think of it as fate, its frequent occurrence does not mean it is unsolvable. Please do not isolate yourself from social life because of a complaint that can be very easily treated.
